A comprehensive framework for MSW practicum students to observe clinical sessions conducted by an experienced LCSW, with structured de-identified analysis and full compliance safeguards.
Everything in this agreement explained in everyday language — no legal jargon required.
This agreement sets up a structured way for graduate social work students to observe real clinical therapy sessions led by an experienced Licensed Clinical Social Worker (LCSW). The student watches, learns, and then writes a private, de-identified reflection about what they observed — all under strict supervision.
Any suspected breach — even if uncertain — must be reported within 24 hours. The activity stops immediately. A written incident report is due within 72 hours, and a corrective action plan within 10 business days. The Clinical Supervisor can halt any observation instantly if needed.
This program bridges the gap between classroom learning and real-world clinical practice. Students gain supervised exposure to professional techniques, ethical decision-making, cultural responsiveness, and assessment methods — all while maintaining the highest standards of client protection and privacy.
The signed agreement, incident reports, and key documents are retained for at least 7 years. Reflective audits are kept for the practicum duration plus 3 years. All records are securely destroyed when their retention period ends, with destruction logged.
At-a-glance reference card — print it, bookmark it, keep it handy.
Every deadline below is measured from the triggering event (e.g., end of a session, discovery of an incident). All timelines are firm obligations — missing one may result in corrective action.
After each shadowing session, the student must complete a de-identified written reflection connecting observations to social-work competencies. Submitted to the Clinical Supervisor for review and feedback.
If any breach of confidentiality, boundary violation, safety concern, or protocol deviation occurs (or is suspected), the involved party must verbally notify both the Clinical Supervisor and Program Lead. Do not wait for certainty — report first.
Following verbal notification, a formal written report must be filed documenting what happened, who was involved, what actions were taken, and what follow-up is needed. This report is retained in compliance records.
After a confirmed incident, the Clinical Supervisor and Program Lead develop a corrective action plan outlining remedial steps, additional training, supervision changes, or access restrictions. The student must acknowledge and follow the plan.
A quarterly audit of all shadowing activities, consent records, reflective submissions, and compliance documentation. Ensures processes remain aligned with ethical guidelines, state licensing requirements, and organizational policies.
Full program review including all agreements, supervisor credentials, university approvals, insurance verification, and accumulated student evaluations. All parties must re-certify their understanding of and commitment to the agreement.
All signed agreements, consent forms, reflective audits, incident reports, corrective action plans, and review records must be securely retained for a minimum of seven years from the date of creation, in accordance with professional record-keeping standards.
Don't wait to be sure — report anything that might be a problem.
